The world of mental health is constantly evolving, and with the rise of artificial intelligence (AI), we’re presented with exciting new tools to support our well-being. As a psychotherapist, I understand the apprehension many of my colleagues feel about AI – the fear of the unknown, and perhaps even the fear of replacement. However, I believe that when understood and utilized thoughtfully, AI can be a powerful ally in your mental health journey, particularly when integrated with therapeutic journaling and using AI to journal between sessions.

I’ve witnessed firsthand how AI-powered journaling can unlock profound insights and facilitate deep therapeutic work between sessions for clients who previously struggled with traditional journaling. It’s not about replacing the invaluable human connection of therapy, but rather augmenting it, providing a unique avenue for self-exploration and processing.

Prior to my work as a psychotherapist, I worked in administration and operations, first in private equity banking and then manufacturing. In those earlier professional spaces, I witnessed first-hand how pivoting was not only necessary, but essential for survival of the businesses. We had to follow technological upgrades and changes or we would go out of business. Unfortunately, many mental health counselors and psychotherapists don’t have that perspective and, instead, fear these technological advances. Rather than fear them, we are better off leaning into the discomfort of change and integrating them (after all, this is exactly what we advocate for with our clients).

  • Accessibility and Consistency: AI is always available, offering a consistent space for reflection whenever inspiration strikes or emotions demand attention. This can be especially beneficial for individuals who struggle with the discipline of regular journaling.
  • Prompting and Guiding: AI can offer prompts, ask probing questions, and even suggest different perspectives, helping you delve deeper into your thoughts and feelings. This can be particularly helpful for those new to journaling or feeling stuck.
  • Pattern Recognition and Insights: Over time, AI can help identify patterns in your entries, highlighting recurring themes, emotional triggers, or shifts in your mood. This can provide valuable data for both you and your therapist to explore.
  • Safe and Non-Judgmental Space: For some, expressing vulnerabilities to an AI feels less intimidating than to another human. This can be a stepping stone to opening up more freely in therapy.
  • Pre-Therapy Exploration: AI journaling can help you articulate and organize your thoughts before a therapy session, making your time with your therapist more focused and productive.

Cons and Important Considerations:

  • Lack of Empathy and Understanding: AI is a language model; it does not possess genuine empathy, understanding, or lived experience. It cannot truly “feel” what you’re going through.
  • Privacy and Data Security: It’s crucial to be aware of the privacy policies of any AI platform you use. Understand how your data is stored and utilized. Read the fine print!!!
  • Misinterpretation: AI might occasionally misinterpret the nuances of your language or emotions, leading to less accurate insights.
  • Over-reliance: The biggest pitfall is the potential for over-reliance, where individuals might mistakenly believe AI can replace professional mental health counseling.
  • Garbage In, Garbage Out: As my more technical clients understand, the quality of AI’s output is directly related to the quality of the input. Providing vague or limited information will yield less helpful responses.

Ideas and Instructions for Therapeutic Journaling with AI

For those ready to explore this innovative approach, here are some ideas and instructions to get started. The key to success often lies in treating AI as a sophisticated tool, much like a helpful assistant, rather than a sentient being.

  1. Start with Specific Prompts:
    • Instruction: Begin by explicitly stating what you want to journal about. For example: “I want to explore my feelings about a recent conflict with a friend. Prompt me with questions that help me understand my role and emotions.”
    • AI’s Role: The AI can then provide targeted questions like, “What were your initial reactions?” “What unsaid expectations might have contributed to the conflict?” or “How did you express your feelings, or not express them?”
  2. Use AI for Reframing Negative Thoughts:
    • Instruction: Journal about a negative thought pattern you’re experiencing. Then, ask the AI to help you reframe it. Example: “I’m constantly thinking, ‘I’m not good enough.’ Can you help me reframe this thought into something more constructive or realistic?”
    • AI’s Role: The AI might offer alternative perspectives, challenge the evidence for the thought, or suggest focusing on strengths or past successes.

    • Instruction: Describe a situation or relationship dynamic. Then, ask the AI to help you consider it from different angles. Example: “I’m feeling frustrated with my colleague’s communication style. Can you help me imagine how they might be perceiving the situation?”
    • AI’s Role: The AI can prompt you to consider your colleague’s potential motivations, stress factors, or communication habits, fostering empathy and understanding.
  4. Process Difficult Emotions:
    • Instruction: Express your raw emotions about a challenging experience. Then, ask the AI to help you identify the underlying needs or feelings. Example: “I’m feeling overwhelmed and angry about my workload. What deeper emotions might be beneath this anger?”
    • AI’s Role: The AI might suggest exploring feelings of powerlessness, fear of failure, or a need for better boundaries.
  5. Summarize and Reflect:
    • Instruction: After a lengthy journaling session, ask the AI to summarize your main points or identify recurring themes. Example: “Based on my entries today, what are the key takeaways or patterns you’ve observed?”
    • AI’s Role: The AI can condense your thoughts, highlight significant insights, or point out areas for further exploration.

    • Instruction: Describe a conversation you need to have and your anxieties around it. Ask the AI to simulate the other person’s potential responses. Example: “I need to talk to my partner about our finances, and I’m nervous they’ll get defensive. Can you play the role of my partner and respond to my opening statement: ‘I’m worried about our spending habits’?”
    • AI’s Role: The AI can generate potential reactions, allowing you to practice your responses and anticipate challenges in a safe space.

The Indispensable Role of Mental Health Counseling and your Psychotherapist or Counselor

It is paramount to reiterate: AI is a tool, not a therapist. While AI journaling can be a profound support, it is not a substitute for professional mental health counseling. Your therapist offers:

  • Human Connection and Empathy: The genuine understanding, validation, and therapeutic relationship that AI simply cannot replicate.
  • Clinical Expertise: A trained therapist can diagnose, develop personalized treatment plans, and provide evidence-based interventions tailored to your unique needs.
  • Ethical Framework: Therapists operate within strict ethical guidelines, ensuring your safety, confidentiality, and well-being.
  • Nuanced Understanding: A therapist can pick up on subtle cues, body language, and vocal inflections that AI will never perceive.
